2011 FDM Education Conference Scholarship Application

Has your Department experienced a reduction in training funds? Would you like to send someone to FDM's Annual Education Conference? If so, please consider applying for this year's scholarship.

FDM is awarding a scholarship to one client again to attend this year's Conference. If you would like your Department to be considered, please fill out the form below with your contact information, along with a brief (100 word) explanation of why you think your Department should be selected.

The scholarship covers all Conference and training fees excluding travel, food and accommodation with the exception of the following on Thursday, October 20th: breakfast, lunch and FDM's Client Appreciation Dinner Wedesday, October 19th.
